Unless your shoe size is wildly out of range of our American folk (I'm guessing 12.5 or 14), you should get whatever size you would get if you bought shoes made for North Americans. I've never heard of anyone having to go up a size for these.
Whether you should follow the American Standard or the Euro conversion depends on what country you live in. If you have clear knowledge of your shoe size in either metric or centimeters, then sticking with your American standard is probably best.
